On or about May 9, 2023, located at 2A Fiber Line Roll Station 1, the employer did not utilize procedures for the control of hazardous energy when employees were setting up the line by threading through powered rollers by hand, exposing the employees to caught-in/caught-between and ingoing nip points.

29 CFR  1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence:  On or about May 9, 2023, and at times prior, at the 2A Fiber Line Roll Station, employees were exposed to machine hazards while setting up the line. The employer did not implement energy control application steps as the machine was not shut down or turned off to perform set up work.

29 CFR  1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks.  (a) On or about May 8, 2023, located at 2A Fiber Line Roll Station 1, employees cut and removed wrapped fibers from rotating powered rollers by reaching around or under a movable barrier guard, exposing the employees to caught-in hazards.   (b) On or about May 8, 2023, located at 2A Fiber Line Roll Station 1, employees removed fibers from the floor under the rotating powered rollers exposing the employees to caught-in hazards.
