29 CFR  1904.41(a)(1): An establishment with 250 or more employees at any time during the previous calendar year, and this part required the establishment to keep records, did not electronically submit information from OSHA Form 300A Summary of Work-Related Injuries and Illnesses to OSHA or OSHA's designee:   a) Toledo Molding and Die LLC located at 515 E. Gypsy Lane Rd Bowling Green, OH: On or about March 2, 2023, the employer failed to submit the company's OSHA Form 300A Summary of Work-Related Injuries and Illnesses, for the 2022 calendar year to OSHA's Injury Tracking Application (ITA). The employer has an average of 350 employees at that plant requiring them to submit the OSHA 300A information to OSHA by March 2, 2023. Failure to keep updated logs and providing the documents to OSHA could inhibit the employer from identifying trends related to occupational injuries or illnesses at their workplace.
