29 CFR  1910.22(a)(1):  All places of employment, passageways, storerooms, service rooms, and walking-working surfaces are kept in a clean, orderly, and sanitary condition.  a) Production Area; on or about  July 10, 2023, the employer exposed workers to slip hazards, in that the warehouse floors were not maintained free from liquid spills to ensure safe pedestrian transit.

29 CFR  1910.132(a):Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered:  (a)  Production Area;  On or about July 10, 2023, employees were exposed to amputation hazards while attempting to use the die grinder when cutting materials, the employer did not require employees to wear personal protective equipment including eye protection and proper hand protection.
