29 CFR 1910.184(c)(6): Slings were not securely attached to their loads:  a) Rig 318 at a temporary worksite located at 39.947395, -108.339182: On or about July 13, 2023, employees engaged in material handling using a steel alloy, chain and link sling, were exposed to struck-by hazards in that the sling was not securely attached to the load. The sling being used to move an over-shot pipe was not securely attached, allowing the over-shot pipe to be released from the sling, striking an employee.

29 CFR 1910.184(e)(1): Alloy steel sling(s) did not have permanently affixed durable identification stating size, grade, rated capacity, and reach:   a) Rig 318 at a temporary worksite located at 39.947395, -108.339182: On or about July 13, 2023, employees are exposed to struck-by hazards in that steel slings used in material handling operations were used without a permanently affixed identification tag.  Employees were exposed when a steel chain sling was used to lift an over-shot casing with a steel alloy chain sling, the rigging equipment lacked any identification that stated the capacity or reach.
