29 CFR  1918.22(e): The employer did not provide a net or other suitable protection where the gangway overhung the water so that there was danger of employees falling between the ship and the dock.  Port of Los Angeles, Berth 126, M/V MSC Ornella:  On and/or before August 16, 2023, the employer did not ensure the Gangway Safety Net was in a serviceable condition.  The Gangway Safety Net had multiple large holes and was not properly secured to the dock creating a gap between the vessel and dock which potentially exposed employee(s) to head and limb injuries from falls. Employer(s) were exposed to damaged gangway safety netting.

29 CFR  1918.22(i): Handrails and walking surfaces of gangways were not maintained in a safe condition to prevent employees from slipping or falling.  Port of Los Angeles, Berth 126, M/V MSC Ornella: On and/or before August 16, 2023, the employer did not provide a handrail and mid-rail to extend towards the bottom of the gangway. This condition potentially exposed employees to fall hazard(s).
