29 CFR  1910.303(b)(2):  Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling.  a) Maintenance Facility:  On or about October 4, 2023, the door on a 120/208v energized electrical panel was not closed exposing employees to a fire and electrical shock hazard.  b) Maintenance Facility:  On or about October 4, 2023, a relocatable power tap (surge protector) was permanently mounted to the wall subjecting employees to fire and electrical shock hazards.
