29 CFR 1910.178(l)(3)(i)(E): Powered industrial truck operators shall receive initial training in the following topics, except in topics which the employer can demonstrate are not applicable to safe operation of the truck in the employer's workplace: Steering an maneuvering. On or about October 20, 2023, employees performing scanning/picking duties while operating powered industrial vehicles in Aisle D426 had to navigate around an empty bin placed at the entrance of the aisle and the employer did not ensure the aisle was kept clear of obstructions.

29 CFR 1910.178(n)(1): All traffic regulations were not observed. A safe distance was not maintained approximately three truck lengths from the truck ahead, and the truck was not kept under control at all times: On or about October 20, 2023, an employee performing scanning duties was struck by another powered industrial vehicle and the employer did not ensure operators were able to keep control of their vehicle in aisles when speed, blind spots, obstacles, pedestrians, and other vehicles had to be maneuvered around.
