29 CFR  1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been  taken out of service until restored to safe operating condition(s).  On or about September 8, 2023, at the yard of facility an employee was operating a forklift in need of repair such as but not limited to:  a.	Cracked windshield on Hyundai forklift Model: 70D-9 which was not taken out of service.  b.	Forklift that did not have seatbelt repaired on Hyundai forklift Model: 70D-9.

29 CFR  1910.334(a)(2)(i):Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket):  On or about November 8, 2023, at the lid area of facility employees were exposed to electrical hazards as there were two angle grinders that had frayed cords and were not visually inspected.
