At the mixer area, located at 1920 Longhorn Ave, Lakeland, FL: On or about 2/13/2024, the employer exposed employees to crushing, struck-by, caught-in and between hazards in that, the employer did not establish energy control procedures specific to each type of machine which may be Locked or Tagged out, nor was the lockout device under the exclusive control of the authorized employee performing the servicing or maintenance.
