29 CFR 1910.28(b)(1)(i): Except as provided elsewhere in this section 29 CFR 1910.28, the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the systems described in 29 CFR 1910.28(b)(1)(i)(A)-(C): On or about November 10, 2023, employees engaged in well servicing activities at the oil and gas lease site were exposed to fall hazards while working on the tubing board in excess of four feet.

29 CFR 1910.140(c)(17): 29 CFR 1910.140(c)(17): The employer did not ensure that a personal fall protection system or its components subjected to impact loading is removed from service immediately and not used again until a competent person inspects the system or components and determines that it is not damaged and safe for use for employee personal fall protection. On or about November 10, 2023, employees engaged in well servicing activities at the oil and gas lease site were exposed to fall hazards while using a personal fall protection system that had been subjected to impact loading and had not been removed from service immediately.
