29 CFR  1928.57(a)(6): At the time of initial assignment and at least annually thereafter, the employer did not instruct every employee in the safe operation and servicing of all covered equipment with which he was or would be involved:  a. Hillandale Farms doing business as Hillandale Farms Ohio, LLC located at 9715 Rosedale Road in Hicksville, Ohio 43526: On or about December 6, 2023,  the employer did not effectively instruct employees in the safe operation and maintenance of equipment to include but not limited to the "Cage Free 90" conveyor. Employees performing operational and service tasks on the aforementioned equipment were not equipped with the information necessary to identify and avoid hazards associated with machine guarding and energy control.    Hillandale Farms doing business as Hillandale Farms Ohio, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR  1928.57(a)(6), which was contained in OSHA inspection number 1716202, citation number 1, item number 4 and was affirmed as a final order on November 14, 2022, with respect to a workplace located at 9715 Rosedale Road in Hicksville, Ohio 43526.

29 CFR  1928.57(c)(2)(i): The mesh or nip-points of all power driven gears, belts, chains, sheaves, pulleys, sprockets and idlers were not guarded:  a. Hillandale Farms doing business as Hillandale Farms Ohio, LLC located at 9715 Rosedale Road in Hicksville, Ohio 43526:  On or about December 6, 2023, the employer did not effectively guard nip-points of belts and rollers located on the cage free house belt conveyor.  Employees performing cleaning duties of the unguarded belts and rollers were exposed to caught in hazards.   Hillandale Farms doing business as Hillandale Farms Ohio, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR  1928.57(c)(2)(i), which was contained in OSHA inspection number 1716202, citation number 1, item number 5b and was affirmed as a final order on November 14, 2022, with respect to a workplace located at 9715 Rosedale Road in Hicksville, Ohio 43526.
