29 CFR  1910.178(l)(3)(i)(G):  Powered industrial truck operators did not receive initial training in the fork and attachment adaptation, operation, and use limitations:  A) 186 Leesville Ave,  Avenel NJ  07001, outside yard;  An Industrial Toolz ramp clamp model # YR208636L attached to the forks of  a powered industrial truck was used to elevate and position yard ramps behind trailers and assist with unloading activities.   Forklift training provided to employees did not include instruction regarding proper and safe use of the ramp clamp.  On or about 12/28/2023 workers installed the ramp clamp upside down, creating an unsafe condition that resulted in the ramp clamp falling onto a worker.   Condition occurred on or about 12/28/23.

29 CFR  1910.178(m)(2):  No person  is allowed to stand or pass under the elevated portion of any truck, whether loaded or empty:  A)  186 Leesville Ave,  Avenel NJ  07001  An employee was exposed to a crush by and struck by hazard while working underneath a elevated ramp that was supported by a  ramp clamp attached to a forklift.  The ramp clamp dislodged and the ramp fell on top of the employee crushing him to death.        Incident occurred on 12/28/23.
