29 CFR  1910.147(d)(5)(i): All potentially hazardous stored or residual energy was not relieved, disconnected, restrained or otherwise rendered safe after the application of lockout or tagout devices to energy isolating devices:  a) On or about 01/18/2024 on the LANGSTON TITAN Rotary Die Cutter (machine number 4561/784); where potential energy from a raised weight (the die cut drum) was not restrained before disconnecting the bolts which secured the die cut drum to the machine frame and prying the die cut drum out of the frame of the machine.  The die cut drum was attached to the "free wheeling" print down with rigging straps and a chain fall. When the die cut drum was rolled off the polyurethane anvil drum, it pulled the print down forward, creating a crushing action between the stationary die cut stand and the free wheeling print down. Employees were exposed to the crush point between the two parts of the machine.  ABATEMMENT DOCUMENTATION REQUIRED
