Domtar Paper Company, LLC, C10 Conveyor area, on or about January 25th, 2024, and times prior thereto: Not all contract employees were adequately informed of the hazards and required precautions necessary for safely entering and working in the C10 conveyor tunnel areas. Contract employees entered the C10 conveyor tunnel to conduct chip vacuum operation without precautions and were exposed to Hydrogen Sulfide, resulting in loss of consciousness, rescue, and emergency medical treatment.     ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation.  This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement. IN ADDITION, abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic, or video evidence or abatement or other written records.

OSH ACT of 1970 Section (5)(a)(1):  The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that site contractor's employees were exposed to Hydrogen Sulfide:  a.) Domtar Paper Company, LLC,: The employer's contractor failed to provide training on the hazards of Hydrogen Sulfide in the C10 conveyor tunnel area.  ABATEMENT NOTE: Among other methods: feasible and acceptable means of abatement include: 1).  Comply with requirements noted in ANSI/ASSP Z390.1 Hydrogen Sulfide Training and the Center for Chemical Process Safety's Golden Rules of Process Safety for Hydrogen Sulfide. 2). Ensure contractors who are potentially exposed to Hydrogen Sulfide are trained on ANSI/ASSP Z390.1 and the Center for Chemical Process Safety's guidelines on Hydrogen Sulfide training. 3). Institute a certification process that ensures the contractor's Hydrogen Sulfide training can be verified before they enter plant grounds and/or before they start work operations.  ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation.  This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.
