Double R Sharpening, on or about February 22, 2024 at the address of 18915 Brake Shoe Road, Meadville, PA: Employees were using an A-frame style ladder to access the second story storage area. The ladder was not used for its intended purpose, in that the ladder did not extend 3 feet above the landing area. Employees had to use the top rung of the ladder to access the storage area.
