29 CFR 1926.453(b)(2)(v):A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: a) Horizon Structures LLC. On or about April 16, 2024 at the address of 5686 E. Harbor Rd. Unit G3. Marblehead, OH 43440. The employer did not ensure that aerial lift operator donned and secured their personal fall arrest systems before operating Genie S-40/Genie S-65 aerial lifts. Employee utilizing aerial lift to performed steel erection work activities (installing standing seam metal roof) was exposed to fall hazards of 23 feet to the ground below.

29 CFR 1926.760(a)(1):Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems. a) Horizon Structures LLC. On or about April 16, 2024 at the address of 5686 E. Harbor Rd. Unit G3. Marblehead, OH 43440. The employer did not ensure employees were protected from hazards associated with falls while working atop a steel frame structure. Five employees performing steel erection work activities (installing standing seam metal roof) were exposed to fall hazards of 23 feet to the ground below.
