29 CFR  1926.651(c)(2):Means of egress from trench excavations. A stairway, ladder, ramp or other safe means of egress shall be located in trench excavations that are 4 feet (1.22 m) or more in depth so as to require no more than 25 feet (7.62 m) of lateral travel for employees.  On or about April 18, 2024, during the relocation of an existing waterline at a worksite, known as the Irish Town Bend Project, located North of 1101 West 25th Street, Cleveland, Ohio, the employer failed to ensure an employee working inside a trench, with a varying depth of approximately 5.7 feet (West end) to approximately 10 feet (East end), was provided with a safe means of egress such as a stairway, ladder, and/or ramp.
