29 CFR  1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:   On or about April 16, 2024, the employer did not ensure that energy control application steps are implemented prior to allowing employees to clean or remove accumulated ink inside the Printing Machine#10.  The machine was not deenergized prior to engaging in these activities, exposing employees to amputation hazards.  Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR  1904.29(b)(3): Each recordable injury or illness was not recorded on the OSHA 300 Log and 301 Incident Report within seven calendar days of receiving information that a recordable injury or illness has occurred:   On or about April 24, 2024, the employer did not record a workplace injury on the OSHA 300 log. An employee sustained a lost time injury on April 16, 2024 that was not recorded on the employer's  OSHA 300 form in a timely manner.
