(i) Except as provided elsewhere in this section, the employer must ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by conventional fall protection (safety nets, guard rails or Personal Fall Arrest Systems).  On or about May 25, 2024, at the facility located at 1328 Indiana Avenue, Sheboygan, Wisconsin, the employer failed to provide guardrails on a mezzanine, exposing employees to a fall hazard of approximately 7 feet 9 inches.

29 CFR 1910.305(g)(1)(iv)(A): Unless specifically permitted otherwise in paragraph (g)(1)(ii) of this section, flexible cords and cables may not be used as a substitute for the fixed wiring of a structure.  On or about May 24, 2024, at the facility located at 1328 Indiana Avenue, Sheboygan, Wisconsin; the employer allowed flexible cords (extension cord) to be used as a substitute for fixed wiring, exposing employee to potential contract with electrical current.  An extension cord was used to power open signs.

1910.1200(f)(6)(ii)  Product identifier and words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals, and which, in conjunction with the other information immediately available to employees under the hazard communication program, will provide employees with the specific information regarding the physical and health hazards of the hazardous chemical. 29 CFR  1910.1200(f)(6): Except as provided in paragraphs (f)(7) and (f)(8) of this section, the employer shall ensure that each container of hazardous chemicals in the workplace is labeled, tagged or marked with either: (i) The information specified under paragraphs (f)(1)(i) through (v) of this section for labels on shipped containers; or, (ii) Product identifier and words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals, and which, in conjunction with the other information immediately available to employees under the hazard communication program, will provide employees with the specific information regarding the physical and health hazards of the hazardous chemical.  On or about May 24, 2022, at the facility located at 1328 Indiana Avenue, Sheboygan, Wisconsin; the employer did not ensure that each container of hazardous chemical was labeled, tagged or marked, with the identity and/or hazards of its contents, exposing an employee to chemical hazards.  Bulk oil containers were not properly labeled, tagged, or marked.
