29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  a) Flowers Foods, Inc. located at 4300 South 26th Street in Philadelphia, Pennsylvania- On or about June 18, 2024, the employer did not ensure lockout/tagout procedures were utilized while performing cleaning maintenance on a conveyor system on the L-30 Transfer Conveyor.  An employee performed cleaning maintenance on a conveyor without de-energizing the system, thereby exposing employee to caught-in between hazards.

29 CFR 1910.147(c)(7)(i): The employer did not provide training to ensure that the purpose and function of the energy control program was understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls were acquired by employees:  a) Flowers Foods, Inc. located at 4300 South 26th Street in Philadelphia, Pennsylvania- On or about June 18, 2024, the employer did not ensure that an employee performing cleaning maintenance on the L-30 Transfer Conveyor was trained on the safe application and usage of the energy controls and understood the purpose and function of the energy control program.
