29 CFR 1910.304(b)(3)(ii)(A): All 125-volt, single-phase, 15, 20 and 30-ampere receptacle outlets that are not part of the permanent wiring of the building or structure and that are in use by personnel did not have ground-fault circuit-interrupter protection for personnel. a) Old Admin Barge, lower hull, East of carpentry shop: On or about June 25, 2024, and at times prior, the employer exposed employees to electrical shock hazards, in that GFCI equipped receptacles were removed and replaced with non-GFCI receptacles where sump pumps used to remove water from the barge are checked daily by employees. b) Old Admin Barge, shed, North side of barge by dry dock: On or about June 25, 2024, and at times prior, the employer exposed employees to electrical shock hazards, in that GFCI equipped receptacles were removed and replaced with non-GFCI receptacles where sump pumps used to remove water from the barge are checked daily by employees. c) Old Admin Barge, lower hull, West side below material storage area: On or about June 25, 2024, and at times prior, the employer exposed employees to electrical shock hazards, in that GFCI equipped receptacles were removed and replaced with non-GFCI receptacles where sump pumps used to remove water from the barge are checked daily by employees.

29 CFR 1915.81(b)(1)(iv): The employer did not place each hose and cord in a location that would prevent injury to employees and damage to the hoses and cords. a) Facility: On or about June 25th, 2024, and at times prior, the employer exposed employees to trip and fall hazards, in that welding hoses were placed over a stairway used to access the vessel in the drydock. First Marine was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1915.81(b)(1)(iv), which was contained in OSHA inspection number 1292252, citation number 1, item number 5 and was affirmed as a final order on January 30, 2020, with respect to a workplace located at Calvert City, Kentucky.
