29 CFR 1910.212(a)(3)(iii): Special hand tools were not used for placing and removing material to permit safe handling of material without the operator placing a hand in the danger zone. Such tools shall not be in lieu of other guarding required by this section, but can only be used to supplement protection provided:  a)Wilson Trailer Company, at 3602 Broadway Avenue, Yankton, SD 57078: On or about and at times prior to July 1, 2024, the employer did not ensure that special tools or other means were utilized for placing and/or removing materials from the ByStronic ByBend Star 80/1530 hydraulic press brake, serial number 23515083. This condition exposed the employee to a danger zone hazard associated with bending product and "flip-up" of the product to the top ram, which resulted in an amputation.

29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):  (a)Wilson Trailer Company at 3602 Broadway Avenue, Yankton, SD 57078: On or about and at times prior to July 8, 2024, the employer did not ensure that the point of operation on the ByStronic Hydraulic/Pneumatic Press, 80/1530, serial 23515083 was adequately guarded when operated in safety speed/mute mode.  This condition exposed employees to amputation and crushing hazards.  (b)Wilson Trailer Company at 3602 Broadway Avenue, Yankton, SD 57078: On or about and at times prior to July 8, 2024, the employer did not ensure that the point of operation on the Trumpf TrueBend 5085,  serial B0503A0574 was adequately guarded when operated in safety speed/mute mode.  This condition exposed employees to amputation and crushing hazards.  (c)Wilson Trailer Company at 3602 Broadway Avenue, Yankton, SD 57078: On or about and at times prior to July 8, 2024, the employer did not ensure that the point of operation on the Trumpf TrueBend 5320, serial B0509A0437 was adequately guarded when operated in safety speed/mute mode.  This condition exposed employees to amputation and crushing hazards.  (d)Wilson Trailer Company at 3602 Broadway Avenue, Yankton, SD 57078: On or about and at times prior to July 8, 2024, the employer did not ensure that the point of operation on the AccurPress Advantage 300, Serial A1864 was adequately guarded when operated in safety speed/mute mode.  This condition exposed employees to amputation and crushing hazards.
