29 CFR  1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:  a) On September 20, 2024, at the above addressed jobsite, employees were exposed to struck-by hazard from falling tools, material and debris when working below employees performing roofing work.  The employees were not wearing hard hats.  No abatement certification or documentation is required for this item.

29 CFR  1926.102(a)(1): The employer did not ensure that each affected employee use appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors or potentially injurious light radiation:  a) On September 20, 2024, at the above addressed jobsite, employees were exposed to eye injury while operating pneumatic nail guns and saws without eye protection.     No abatement certification or documentation is required for this item.  Miranda Construction Services, LLC was previously cited for a violation of this occupational safety and health standard 1926.102(a)(1), which was contained in OSHA inspection number 1630847, citation number 1, item number 1 and was affirmed as a final order on December 5, 2022, with respect to a workplace located at 744 North Walnut Lane in Schaumburg, Illinois.

29 CFR  1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   a) On September 20, 2024, at the above addressed jobsite, employees performing roofing work on residential home, approximately 16 feet and 5 inches above the ground below, were not secured to personal fall arrest systems or other means of fall protection, thereby exposing the employees to fall hazards.    b) On September 20, 2024, at the above addressed jobsite, employees performing roofing work on residential home, approximately 16 feet and 10 inches above the ground below, were not secured to personal fall arrest systems or other means of fall protection, thereby exposing the employees to fall hazards.    No abatement certification or documentation is required for this item. 29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) On September 20, 2024, at the above addressed jobsite, the portable ladder used by employees to access and egress the roof of the house, measuring approximately 16 feet five (5) inches from ground to eve, was not extended to have the side rails extend at least three (3) feet above the upper landing.    b) On September 20, 2024, at the above addressed jobsite, the portable ladder used by employees to access and egress the roof of the house, measuring approximately 16 feet 10 inches from ground to eve, was not extended to have the side rails extend at least three (3) feet above the upper landing.    No abatement certification or documentation is required for this item.
