29 CFR  1926.102(a)(1):The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  a) On or about April 24, 2025, worksite 2707 Champlain St., Ottowa, Illinois 61350, employees performing roofing work were exposed to struck-by hazards associated with sharp flying debris while operating pneumatic nail guns without eye protection, likely to cause serious eye injury.     No abatement certification or documentation is required for this item.   Miranda Construction Services, LLC was previously cited for a violation of this occupational safety and health standard 1926.501(b)(13), which was contained in OSHA inspection number 1630847, citation number 1, item number 2 and was affirmed as a final order on December 5, 2022, with respect to a workplace located at 744 North Walnut Lane in Schaumburg, Illinois 60194.  Miranda Construction Services, LLC was previously cited for a violation of this occupational safety and health standard 1926.501(b)(13), which was contained in OSHA inspection number 1776938, citation number 2, item number 2 and was affirmed as a final order on December 9, 2024, with respect to a workplace located at 435 E Deerpath Road in Lake Forest, Illinois 60045.

29 CFR  1926.501(b)(13):"Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure. Exception: When the employer can demonstrate that it is infeasible or creates a greater hazard to use these systems, the employer shall develop and implement a fall protection plan which meets the requirements of paragraph (k) of 1926.502.  a) On or about April 24, 2025, worksite 2707 Champlain St., Ottawa, Illinois 61350, employees were observed working on the upper-level of a residential structure at a height of approximately 18 feet. Employees were performing roofing activities on the upper-level and were exposed to fall hazards of approximately 18 feet because fall protection was not utilized.    No abatement certification or documentation is required for this item.   Miranda Construction Services, LLC was previously cited for a violation of this occupational safety and health standard 1926.501(b)(13), which was contained in OSHA inspection number 1630847, citation number 1, item number 2 and was affirmed as a final order on December 5, 2022, with respect to a workplace located at 744 North Walnut Lane in Schaumburg, Illinois 60194. 29 CFR 1926.503(b)(1): The employer shall verify compliance with paragraph (a) of this section by preparing a written certification record. The written certification record shall contain the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer. If the employer relies on training conducted by another employer or completed prior to the effective date of this section, the certification record shall indicate the date the employer determined the prior training was adequate rather than the date of actual training.    a) On or about April 24, 2025, the employer did not certify that employees had been trained to recognize and minimize fall hazards. Employees were exposed to fall hazards of approximately 18 feet while performing roofing activities without using fall protection.   Abatement certification is required for this item in accordance with 29 CFR 1903.19(c).
