29 CFR 1910.25(b)(5)(i): The employer did not ensure that when a door or a gate opens directly on a stairway, a platform is provided, and the swing of the door or gate does not reduce the platform's effective usable depth to less than 20 inches (51 cm) for platforms installed before January 17, 2017.  Employees are exposed to slip, trip, and fall hazards. This was most recently documented on or about November 14, 2024, at 749 North Street, Lincoln, Nebraska. The door leading from the paint bay to the west shop opened immediately unto a stair-step without a platform.
