29 CFR 1910.147(c)(4)(ii)(B): Energy control procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy: On or about December 18, 2025, at a facility located at 502 E Linden Street in Abbotsford, WI; the employer's energy control procedure titled, "Equipment-Specific Lockout/Tagout Procedure for Sanitation" for the Oven Room 2 Spiral Oven did not clearly and specifically address: a) The specific procedural steps addressing the thermal (heat) energy source by implementing a waiting period or other methods to allow components to cool down prior to sanitation, maintenance, and/or servicing activities. b) The specific procedural steps for shutting down, isolating, and securing either the manual gas valve and/or the Siemens gas valve controlled by the computer system utilized by employees during the sanitation, maintenance, and/or servicing activities. c) The specific procedural steps for shutting down, isolating, and securing the steam energy isolating devices utilized by employees during the sanitation, maintenance, and/or servicing activities.

29 CFR 1910.147(c)(4)(ii)(D): Energy control procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, specific requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures: On or about December 18, 2025, at a facility located at 502 E Linden Street in Abbotsford, WI; the employer's energy control procedure titled, "Equipment-Specific Lockout/Tagout Procedure for Sanitation" did not clearly and specifically outline specific requirements for testing and/or verification of the effectiveness of lock devices and other energy control measures for the Oven Room 2 Spiral Oven. The energy control procedure did not contain specific requirements for: a) The administrative controls for the safe dissipation of heat from the oven. b) Testing and verification of the effectiveness of the lockout of the natural gas energy to the equipment. c) Testing and verification of the effectiveness of the lockout of the steam energy to the equipment.

29 CFR 1910.303(b)(2): Listed or labeled equipment shall be installed and used in accordance with any instructions included in the listing or labeling: On or about December 18, 2025, at a facility located at 502 E Linden Street in Abbotsford, WI; a metallic outlet box (handy box), in the Training Room/Manager's Office, was connected to flexible cords and used as extension cord for computer/equipment charging cords. The metal outlet boxes were not installed in accordance with the UL identifier QCIT, which states that boxes shall be mounted in or on a surface.
