Employees performing tank cleaning tasks from the catwalk where hoses were maintained in a strung condition, were exposed to slip-trip hazards.

29 CFR  1910.132(a): Employer did not provide personal protective equipment to be used when necessary whenever hazards capable of causing injury or impairment were encountered.  a) Lima Tank Wash, Inc. on or about December 23, 2024 at the location 1590 Findlay Road, Lima, Ohio 45801: The employer did not ensure employees were provided with adequate personal protective equipment (PPE) to protect against burns. Employees tasked with work from the mixing deck in bay two and three, in proximity of  unprotected heated pipes were exposed to burn hazards.

Due to this condition employees working on or around forklifts were exposed to potential stuck by hazards.
