Employees securing the plastic wrap to paper rolls were exposed to slips and falls onto the same level as the hazard.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): a) On or about January 28, 2025, at an establishment located at 2441 East Glendale Avenue, Appleton, Wisconsin, the employer did not ensure employees were protected from machine hazards when engaged in servicing and maintenance activities on the print roll on the Vireo 3 Wolverine print unit. The employer did not ensure that employees implemented energy control application steps prior to allowing them to clean the print roll. The machine was not deenergized prior to the employee engaging in the servicing / maintenance activity exposing employees to caught-in hazards. As a result, the remaining applicable energy control elements, involving machine isolation [1910.147(d)(3)], lockout/tagout device application [1910.147(d)(4)], dissipation of residual energy [1910(d)(5)(i)], and verification of isolation [1910.147(d)(6)] were not implemented to protect employees from machine hazards. b) On or about January 28, 2025, at an establishment located at 2441 East Glendale Avenue, Appleton, Wisconsin, the employer did not ensure employees were protected from machine hazards when engaged in cleaning activities on the Vireo 3 Wolverine print unit. The employer did not ensure that employees implemented energy control application steps or equivalent protective measures prior to allowing them to clean the print and anilox rolls. The machine was not deenergized prior to the employees engaging in the servicing / maintenance activity exposing employees to caught-in hazards. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. a) On or about April 8, 2025, at an establishment located at 2239 East Glendale Avenue in Appleton, Wisconsin, in Plant 3 - Skid Shop; the employer did not provide machine guarding to protect employees working near the two Pallet Chief nailer machines to prevent employee exposure to the struck-by hazards created by the flying chips and particles produced by the three pneumatically actuated fastening tools mounted to the carriage of the machine. b) On or about April 7, 2025, to April 11, 2025, at an establishment located at 2239 East Glendale Avenue in Appleton, Wisconsin, in Plant 3 - Skid Shop; the employer did not provide machine guarding to protect employees who worked in pairs constructing skids on the three hydraulic scissors lift tables. Employees were exposed to the caught-between hazard created between the concrete floor and the lowering tables. c) On or about April 9, 2025, at an establishment located at 2239 East Glendale Avenue in Appleton, Wisconsin, in Plant 3 - Skid Shop; the employer did not provide machine guarding to protect employees operating Notcher #1 from the caught-between hazards to the hands and fingers created by the pneumatic workpiece clamp.

Guards shall be affixed to the machine where possible and secured elsewhere if for any reason attachment to the machine is not possible. The guard shall be such that it does not offer an accident hazard in itself. On or about April 9, 2025, at an establishment located at 2239 East Glendale Avenue in Appleton, Wisconsin, in Plant 3 - Skid Shop; the pop-up guard on Notcher #1 created a caught-between hazard when it returned to the home position after the machine cycled. The upward movement of the guard created an approximate 1/2-inch gap between the guard and the workpiece clamp when employees reached in to remove wood boards.

29 CFR 1910.213(h)(1): Radial saws. The upper hood shall completely enclose the upper portion of the blade down to a point that will include the end of the saw arbor. The upper hood shall be constructed in such a manner and of such material that it will protect the operator from flying splinters, broken saw teeth, etc., and will deflect sawdust away from the operator. The sides of the lower exposed portion of the blade shall be guarded to the full diameter of the blade by a device that will automatically adjust itself to the thickness of the stock and remain in contact with stock being cut to give maximum protection possible for the operation being performed. On or about April 8, 2025, at an establishment located at 2239 East Glendale Avenue in Appleton, Wisconsin, in Plant 3 - Skid Shop; employees were exposed to point-of-operation hazards while utilizing the DeWalt radial arm saw (Model: GE, Serial: 74669) to cut notches in lumber for pallets when the guard intended to cover the sides of the lower portion of the blade was stuck in the up position. The lower portion of the blade was not guarded by a self-adjusting guard.

29 CFR 1910.305(g)(1)(iv): Unless specifically permitted otherwise in paragraph (g)(1)(ii) of this section, flexible cords and cables may not be used for purposes prohibited by paragraphs (A) through (F) of 29 CFR 1910.305(g)(1)(iv): 29 CFR 1910.305(g)(1)(iv)(A): As a substitute for the fixed wiring of a structure; a) On or about April 8, 2025, at an establishment located at 2239 East Glendale Avenue in Appleton, Wisconsin, Plant 3 - Skid Shop; an extension cord was used as a substitute for the fixed wiring of a structure when it provided 120-vac electrical power from an outlet to a 1 horsepower Baldor motor that was used in part to supply electrical power for the actuation of the Presto hydraulic scissor lift table. The extension cord was wrapped around fixed airline pipes mounted to the building's structure, and routed up and over a walkway, exposing employees to electrical fire and shock hazards. b) On or about January 28, 2025, at an establishment located at 2441 East Glendale Avenue in Appleton, Wisconsin, Plant 2 breakroom; an extension cord was used as a substitute for the fixed wiring of a structure when it was series connected to a relocatable power tap to provide power from an 120-vac outlet to an air fryer oven and air fryer. The extension cord was wrapped around fixed piping, and routed up and around electrical panels and furniture, exposing employees to electrical fire and shock hazards.

The employer is failing to ensure employees engaged in servicing and maintenance activities were trained to the authorized level to safely control the machine's hazardous energy prior to performing work. Machine Operators are exposed to caught-in hazards of the machine's moving/rotating parts while performing servicing and maintenance activities such as, but not limited to, cleaning rollers, inspecting, and making adjustments to machine components on the Vireo 3 print machine. This was most recently documented on January 28, 2025, in Plant 2 which is located at 2441 East Glendale Avenue, Appleton, Wisconsin. Progressive Converting, Inc. DBA Pro-Con was previously cited for a violation of a similar Occupational Safety and Health Standard 29 CFR 1910.147(c)(7)(i), which was contained in OSHA inspection number 1744903, citation 1, item 2, and was affirmed as a final order on October 10, 2024, with respect to a workplace located at 2430 East Glendale Avenue, Appleton, Wisconsin.

The was most recently documented on January 28, 2025, in the Plant 2 break area behind the Shipping Office, which is located at 2441 East Glendale Avenue in Appleton, Wisconsin, Progressive Converting, Inc., dba ProCon, was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1910.303(b)(2), which was contained in OSHA inspection 1744903, citation number 3, item number 4, and was affirmed final order on October 10, 2024, with respect to a workplace located at 2430 East Glendale Avenue in Appleton, Wisconsin.
