29 CFR  1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:   a) 3249 Hempland Road, Lancaster, PA - On or about February 18, 2025, Employees performed servicing and maintenance activities, such as cleaning, tool and die changes, and repairs, on a variety of machinery and equipment, including, but not limited to, the 350-ton Niagara hydraulic press brake, the 75-ton Pacific hydraulic press brake, the Cincinnati mechanical shear, and the Niagra mechanical shear, and the employer did not develop, document, and implement machine-specific energy control procedures, exposing employees to hazards, including but not limited to getting caught in and struck by moving machine parts.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.

29 CFR  1910.147(c)(6)(i):The employer did not conduct an annual or more frequent inspection of the energy control procedure to ensure that the procedure and requirements of this standard were followed:  a) 3249 Hempland Road, Lancaster, PA - On or about February 18, 2025, the employer did not conduct periodic inspections of the hazardous energy control procedures or perform authorized personnel evaluations on an annual or more frequent basis.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.

29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation:  a) 3249 Hempland Road, Lancaster, PA - On or about February 18, 2025, machine operators conducted tool and die change activities on equipment, including, but not limited to, the 350-ton Niagara hydraulic press brake and the 75-ton Pacific hydraulic press brake. The employer did not conduct authorized employee training for employees who perform tasks that require an understanding of the methods and measures to be taken when conducting covered activities.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a) 3249 Hempland Road, Lancaster, PA - On or about February 18, 2025, the FMB Phoenix bandsaw used for cutting metal was not guarded to enclose the full blade outside of the point of operation, exposing employees to hazards from the rotating blade.  Corrected during the inspection, no abatement certification or documentation required.
