29 CFR 1910.28(b)(3)(iv):  The employer did not provide that each employee be protected from falling into a ladderway floor hole or ladderway platform hole by a guardrail system and toeboards erected on all exposed sides, except at the entrance to the hole, where a self-closing gate or an offset must be used:  A) Connection Department: On or about March 25, 2025, the employer exposed employees to a fall hazard, in that the gate to the tan platform was not self-closing.   B) Phase Assembly Area: On or about March 25, 2025, the employer exposed employees to a fall hazard, in that the self-closing gate to the yellow platform was tied in the open position.   C) Up End Area: On or about March 25, 2025, the employer exposed employees to a fall hazard, in that there was a chain at the access opening on the tan platform, not a self-closing gate.
