C)	On or about June 4, 2025, in the hole area /rebar holding station, an employee did have have a form of access or egress while conducted rebar installations on top of a rebar holding station above 4 feet from the ground.

B) On or about June 4, 2025, in the hole area, employees conducted final finishing on a concrete cap above 4 feet from the ground without utilizing fall protection.   C) On or about June 4, 2025, in the hole area/rebar holding station, an employee conducted rebar installations while on top of a rebar holding station above 4 feet from the ground without utilizing fall protection.
