29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket): a) Newport News Ship Yard at 4101 Washington Avenue, Newport News, VA 23607; on or about May 1, 2025, employees were exposed to electrocution hazards when the flexible cords were not inspected in a way to locate external defects to the internal wiring.

29 CFR 1910.334(a)(2)(ii): There was a defect or evidence of damage that could have exposed an employee to injury and the defective or damaged item was not removed from service: a) Newport News Ship Yard at 4101 Washington Avenue, Newport News, VA 23607; on or about May 5, 2025, employees were exposed to electrical hazards after it was determined that they were using damaged equipment to conduct/complete welding operations. The damage noted during the walk around was/ but not limited to; exterior insulation with cuts/break, damaged grounds, and equipment modifications.
