Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.

29 CFR 1910.28(b)(13)(ii): The employer did not ensure that when work was performed at least 6 feet (1.6 m) but less than 15 feet (4.6 m) from the roof edge, each employee was protected from falling by using a guardrail system, safety net system, travel restraint system, or personal fall arrest system. The employer may use a designated area when performing work that is both infrequent and temporary: (a) Roofs on Buildings 48, 60, 72, and 87 - Employees frequently went onto roofs to perform inspections, preventative maintenance, and repair work on items, including, but not limited to, the roofing, utilities, and equipment. The employer did not ensure that each employee had protection from falling by a guardrail system, safety net system, travel restraint system, or personal fall arrest system when employees worked at least 6 feet but less than 15 feet from unprotected roof edges, on or about the week of May 27, 2025. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.

29 CFR 1910.29(e)(1): The employer did not ensure that each cover for a hole in a walking-working surface is capable of supporting without failure, at least twice the maximum intended load that may be imposed on the cover at any one time: (a) Building 87 Roof, Cardiology Department - Maintenance technicians access Building 87's roof through an open half door in a roof top level utility chase in a stair tower on the west end of Building 87 and walk between or along the sides of two side-by-side rows of skylights to perform inspections, preventative maintenance, and repair work of the roof top equipment, utilities, and structures located on the roof. The skylights' glazing, which comprised the only cover, did not have a capacity rating for fall resistance (i.e., capable of supporting without failure, the weight imposed by a falling employee), and maintenance technicians did not use any other fall protection system, on or about the week of May 27, 2025. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.
