29 CFR 1904.32(b)(3): The company executive did not properly certify that he or she examined the OSHA 300 Log and that he or she reasonably believed, based on his or her knowledge of the process by which the information was recorded, that the annual summary was correct and complete:  (a) Ephrata Community Hospital, 169 Martin Avenue, Ephrata, Pennsylvania - The company executive did not properly certify the 2025 calendar year OSHA 300A in that totals on this 300A did not match totals on the 2025 calendar year 300 log.  For example, the 2025 calendar year 300A listed 43 as the number of deaths for column G while the 300 log showed zero deaths, and the 2025 calendar year 300A listed 273 as the total number of days away from work for column K while the 300 log added up to 323 days away from work, on or about January 12, 2026.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.
