29 CFR  1926.502(d)(15): Anchorages used for attachment of personal fall arrest equipment shall be independent of any anchorage being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee attached, or shall be designed, installed, and used as follows:  Alexander Hernandez DBA A&A Roofing & Siding LLC does not require the use of fall protection during roofing operations. This violation was most recently observed on June 2, 2025, at a commercial re-roofing project located at 2580 West 9th Avenue in Oshkosh, Wisconsin. Employees were exposed to falls hazards of 13 feet and greater while tearing off old roofing materials and were not utilizing a designed anchor point capable of supporting 5000lbs.

29 CFR  1926.502(d)(17): The attachment point of the body belt shall be located in the center of the wearer's back. The attachment point of the body harness shall be located in the center of the wearer's back near shoulder level, or above the wearer's head.  Alexander Hernandez DBA A&A Roofing & Siding LLC does not require the use of fall protection during roofing operations. This violation was most recently observed on June 2, 2025, at a commercial re-roofing project located at 2580 West 9th Avenue in Oshkosh, Wisconsin. Employees were exposed to falls hazards of six feet and greater while tearing off old roofing materials  and did not attach their fall arrest components to a point near the center of their back.

29 CFR  1926.501(b)(11):"Steep roofs." Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems.  Alexander Hernandez DBA A&A Roofing & Siding LLC does not require the use of fall protection during roofing operations. This violation was most recently observed on June 2, 2025, at a commercial re-roofing project located at 2580 West 9th Avenue in Oshkosh, Wisconsin. Employees were exposed to falls hazards of 13 feet and greater while tearing off old roofing materials and installing new asphalt shingles without their harnesses being connected to lifelines or anchors.  Alexander Hernandez DBA A&A Roofing & Siding LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1740137, citation number 2, item number 1, and was affirmed as a final order on 05-22-2024, with respect to a workplace located at 738 East Monroe Avenue, Hartford, WI 53027.  Alexander Hernandez DBA A&A Roofing & Siding LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1687713, citation number 1, item number 1a, and was affirmed as a final order on 11-08-2023, with respect to a workplace located at 6550 N Meade St, Appleton, WI 54913.  Alexander Hernandez DBA A&A Roofing & Siding LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1593743, citation number 1, item number 1 and was affirmed as a final order on 09-28-2022, with respect to a workplace located at 1198 Olson Ave, Oshkosh, WI 54901.
