29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  a.  On or about June 12, 2025, at the above addressed jobsite, employees conducting roofing operations were not provided training on how to minimize fall hazards, thereby exposing the employees to the hazards associated with falls.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a.  On or about June 12, 2025, at the above addressed jobsite, employees engaged in roofing activities were not protected from fall hazards while working at an elevation of approximately nine (9) feet, above the ground, thereby exposing employees to fall hazards.  b.  On or about June 12, 2025, at the above addressed jobsite, employees engaged in roofing activities were not protected from fall hazards while working at an elevation of approximately twelve (12) feet, above the ground, thereby exposing the employees to fall hazards.  TRC Construction LLC was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.501 (b)(13), which was contained in OSHA inspection number 1814805, citation number 1, item number 1 and was affirmed as a final order on May 16, 2025, with respect to a workplace located at 203 E Berkshire Ln, Mt. Prospect, IL 60056.  TRC Construction LLC was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.501 (b)(13), which was contained in OSHA inspection number 1752295, citation number 1, item number 2 and was affirmed as a final order on September 20, 2024, with respect to a workplace located at 1717 Willow Ln, Mt. Prospect, IL 60056.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   a.  On or about June 12, 2025, at the above addressed jobsite, the portable ladder used to access the roof did not extend at least three (3') feet above the upper landing surface, thereby exposing employees to the hazards associated with falls.  TRC Construction LLC was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.501 (b)(13), which was contained in OSHA inspection number 1752295, citation number 1, item number 3 and was affirmed as a final order on September 20, 2024, with respect to a workplace located at 1717 Willow Ln, Mt. Prospect, IL 60056.
