29 CFR 1910.22(d)(2): Walking-working surfaces. General requirements. Inspections, maintenance and repair. The employer must ensure: Hazardous conditions on walking-working surfaces are corrected or repaired before an employee uses the walking-working surface again. If the correction or repair cannot be made immediately, the hazard must be guarded to prevent employees from using the walking-working surface until the hazard is corrected or repaired; a) On or about July 16, 2025, the employer did not ensure that a hazardous walking-working surface at loading dock #10 was corrected or guarded, and during the manual repositioning of the dock plate an employee's fingers were caught between the dock plate and the trailer.

29 CFR 1910.30(b)(2):(2) Walking-working surfaces. Training requirements. Equipment hazards. The employer must train each employee who uses a dock-board to properly place and secure it to prevent unintentional movement. a) On or about July 16, 2025, the employer did not ensure that employees were trained on the proper placement and securement of a dock-board, and during the manual repositioning of a misaligned dock plate, the plate fell and caught the employee's fingers between the dock plate and the trailer.
