On or about August 1, 2025, located at the the Gateway Lofts project the employer did not conduct adequate inspections of the scaffolding in that they failed to identify several visible defects including:  absence of stacking pins in the frame of the scaffolding system, absence of the top-tier ladder for ingress/egress the scaffolding, and inadequate fall protection on the third level of the scaffolding system. As a result employees were exposed to a fall hazard of 18' while working from the frame scaffolding.
