29 CFR  1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval:  a) Worksite 971 Waverly Avenue, Holtsville, NY 11742- Employees were exposed to struck by hazards while using a Toyota Power Industrial Truck Model 8FD35U with a chain around the forks to lift a rear differential transfer case out of a vehicle.  The chain was not secured to the forks and the employer did not get prior approval from the manufacturer: on or about September 11, 2025.    Note: The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.

Note: The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.
