29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):  On or about 10/25/2025, in the flowers warehouse area of the establishment located at 3360 NW 67th Avenue, Bldg. 725, Miami, Florida, employees were exposed to struck-by hazards when employees were operating a forklift prior to the employer providing the operator with powered industrial truck (PIT) training to recognize and avoid the hazards associated with the use and operation of PITs.
