29 CFR  1910.178(l)(3)(ii)(A): Powered industrial truck operators did not receive initial training in surface conditions where the vehicle would be operated that could affect safe operation:  Location: a) On jobsite: loading/unloading area at the front of the warehouse:   On or about December 18, 2025, employees who were operating forklifts trucks were not provided with hazard recognition and avoidance training for unsafe surface conditions. Forklift truck operators using the Toyota 50?8FGCU30 equipped with cushion tires operated on wet and slippery snow-covered grounds without receiving training on recognizing or avoiding these surface conditions.
