29 CFR 1915.74(a)(2): The employer permitted employees to board or leave a vessel by using a gangway, and the turntable if used, that did not have on each side a railing with a minimum height of approximately 33 inches, measured perpendicularly from rail to walking surface at the stanchion, with a midrail. Rails shall be of wood, pipe, chain, wire or rope and shall be kept taut at all times. USS Portland, stern of the vessel: On or about January 14, 2026, and at times prior thereto, employee(s) entered and exited the vessel by using a gangway which had a portion of the guardrail(s) missing on each side. This condition potentially exposed employee(s) to fall hazard(s).

29 CFR 1910.134(h)(2)(i): Respirators were not stored to protect them from damage, contamination, dust, sunlight, extreme temperatures, excessive moisture, and damaging chemicals or were not packed or stored to prevent deformation of the facepiece and exhalation valve: Pier 12, next to the USS Portland, PCE toolbox: On or about January 14, 2026, and at times prior thereto, employee(s) used a half face respirator and did not store it to protect it from damage or dust. This condition potentially exposed employee(s) to respiratory hazard(s).
