29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s): (a) Press 4-2, secondary finishing station; On or about February 11, 2026, and dates prior, the service and maintenance tasks of replacing tape rolls and/or clearing jammed tape from the tape cutter/outfeed mechanism was performed on the Start brand ZCM 1000 electronic tape dispenser (serial number 142467) without the equipment being unplugged to isolate it from the energy source.

29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): (a) Press 4-2, secondary finishing station; On or about February 11, 2026, and dates prior, employees were exposed to improperly guarded cutting blades on the Start brand ZCM 1000 electronic tape dispenser (serial number 142467) as the tape cutter outfeed opening was large enough to allow for employees to have their fingers within the zone of danger during operating cycles.
