Employees were working and interacting with the boxes  that had unused openings with electrical power.  a) A receptacle outlet box located near a side table in the office had two unused openings. The box was supplying power to charge sampling instruments and other items.  b) A unused opening was found on the open electrical panel box in the office. The box was powering the entire office.

The employer did not promptly correct any dust collection deficiencies when discovered, and duct tape was used throughout the facility in efforts to control fugitive grain dust.
