American Seafoods F/T American Triumph, Maintenance shop: On or about February 24, 2026, and at times prior thereto, employee(s) used a fixed ladder with less than four inches of toe clearance to access the main deck. The ladder was considered unsafe because there was not adequate clearance behind the rungs. This condition exposed employee(s) to slip, trip and fall hazard(s).
