29 CFR  1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazard(s) created by pinch points, in-running nip points and rotating parts:  On, or about, February 25, 2026, in the following locations, adequate machine safeguarding was not provided:  A.	Edging and Lining:  1.	Machine #1:  An opening in the barrier guard for the glue dispenser allowed access to the point of operation and the rotating handwheel was not safeguarded.  2.	Machine #2:  The rotating handwheel was not safeguarded.  3.	Machine #3:  The rotating handwheel was not safeguarded.  4.	Machine #4:  The rotating handwheel was not safeguarded.  5.	Machine #5:  An opening in the barrier guard that had been used for the glue dispenser allowed access to the point of operation and the rotating handwheel was not guarded.  6.	Machine #6:  An opening in the barrier guard for the glue dispenser allowed access to the point of operation and the rotating handwheel was not safeguarded.  7.	Machine #7:  An opening in the barrier guard for the glue dispenser allowed access to the point of operation and the rotating handwheel was not safeguarded.  B.	Tumbling:  On the following machines, the in-running nip point created by the friction drive was not adequately safeguarded:  1.	Tumbler #1:  A barrier guard was out of place because it was not properly secured to the frame of the machine.   2.	Tumbler #2:  A barrier guard was missing.  3.	Tumbler #4:  The barrier guard provided on the side of the machine allowed access to the contact point of the friction drive.

Personal protective equipment such as, but not limited to, fire resistive clothing, face shields, hard hats and hearing protection was not routinely worn by employees who performed live electrical work, including trouble-shooting, or who verified that electrical equipment was properly de-energized before working on the equipment.
