29 CFR  1910.219(f)(3): Sprocket wheels and chains which were seven (7) feet or less above floors or platforms were not enclosed:  a.) At the worksite, located at  8000 27th Ave N St. Petersburg, FL 33710; On or about 10/22/25,  the employer exposed employees to amputation hazards, in that a Kenworth shingle conveyor truck did not contain a guard on the rotating chain system attached the overhead conveyor system while conducting rooftop shingle offloading operations.

At the worksite, located at  8000 27th Ave N St. Petersburg, FL 33710; On or about 10/22/25,  the employer failed to report a work-related incident, resulting in an employee sustaining an amputation, to OSHA within twenty four (24) hours.
