29 CFR  1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:   Throughout facility: On and prior to April 3rd, 2026,  employee(s) used corrosive chemicals such as ECOLAB's  Rapid Multi Surface Disinfectant Cleaner, Stain Blaster Enzyme Boost, Stain Blaster Grease Remover, Stain Blaster Multi-purpose,73 Disinfecting Acid Bathroom Cleaner, Stain Blaster Destainer, and Stain Blaster Rust Remover.  An emergency eyewash station, providing 15 minutes of flush time, was not provided.   Notice 1 #, Item 2 #, 29 CFR 1910.151(c): The employer failed to ensure employees were not exposed to injurious corrosive materials and suitable facilities for quick drenching or flushing of the eyes and body were provided within the work area for immediate emergency use. The employer was cited for a violation of the same standard 1910.151(c) on March 1, 2024, Inspection # 1724589, date March 28, 2024 of final order/settlement, and means of abatement.  Notice 2 #, Item 1 #, 29 CFR 1910.151(c): The employer failed to ensure employees were not exposed to injurious corrosive materials and suitable facilities for quick drenching or flushing of the eyes and body were provided within the work area for immediate emergency use. The employer was cited for a violation of the same standard 1910.151(c) on January 27th , 2026, Inspection # 1866373, date February 27, 2026 of final order/settlement and means of abatement.

29 CFR  1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure:  Kitchen: On or about April 3rd, 2026, a yellow flexible cord was used as a source of permanent power and was used to power a Keystone window air conditioner.   Notice 1 #, Item 3 #, 29 CFR 1910.305(g)(1)(iv)(A): The employer failed to ensure flexible cords and/or cables were not used as a substitute for the fixed wiring of a structure. The employer was cited for a violation of the same standard 1910.305(g)(1)(iv)(A) on October 13, 2023, Inspection #1699079, date November 14, 2023 of final order/settlement, and means of abatement.  Notice 3 #, Item 2 #, 29 CFR 1910.305(g)(1)(iv)(A): The employer failed to ensure flexible cords and/or cables were not used as a substitute for the fixed wiring of a structure. 29 CFR  1910.157(c)(1): Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries:  Break Room : On or about  April 3rd, 2026, a fire extinguisher was mounted at 5 foot 8 inches in which the top of the handle should have been hung no higher than 5 feet per the requirements of NFPA (National Fire Protection Association) 10, Section 6.1.3.8 for a 40 lbs. or less fire extinguisher.

29 CFR  1904.33(a): The employer did not  save the OSHA 300 Log, the privacy case list (if one exists), the annual summary, and the OSHA 301 Incident Report forms for five (5) years following the end of the calendar year that these records covered.  Inns of the Corps Pendleton - Pacific View Lodge : On or about  April 3rd, 2026,  the OSHA Form 300 and Annual Summary, were not maintained for calendar years 2024 and 2025.
