29 CFR 1910.28(b)(1)(i): Unprotected sides and edges. Except as provided elsewhere in this section, the employer must ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the following: (A) Guardrail systems; (B) Safety net systems; or (C) Personal fall protections systems, such as personal fall arrest, travel restraint, or positioning systems.  On or about May 26, 2026, at an establishment located at 1010 22nd Street in Two Rivers, Wisconsin - East Dock; the employer did not ensure that a truck driver unloading a truck was protected from falling off the west edge of the hydraulic dock platform, exposing the employee to a fall hazard of approximately 4 feet to the concrete ground surface below.

29 CFR 1910.212(a)(1): Machine Guarding. Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are - barrier guards, two-hand tripping devices, electronic safety devices, etc.  On or about May 26, 2026, at an establishment located at 1010 22nd Street in Two Rivers, Wisconsin; employees and truck drivers working near the east and west docks were exposed to crushed-by and sheer point hazards created by the unguarded hydraulic scissor lift platforms of the docks.

29 CFR 1910.334(a)(2)(ii): Visual inspection. If there is a defect or evidence of damage that might expose an employee to injury, the defective or damaged item shall be removed from service, and no employee may use it until repairs and tests necessary to render the equipment safe have been made.  On or about May 26, 2026, at an establishment located at 1010 22nd Street in Two Rivers, Wisconsin, in the east loading dock area; employees were exposed to electrical shock and fire hazards when the employer did not remove from service a flexible extension cord used to charge the Prime Mover electric walkie straddle stacker after it was identified to have damage to the  outer protective sheath, exposing the electrical conductors inside.
