An employee was cleaning on top of the picker when he closed the door on his finger. The middle ring fingertip was removed.

An employee smashed his finger at a job site while helping to hook up a job trailer to a pickup.

An employee lacerated the tip of the left middle finger while using a fillet cutter.

An employee was operating a fork truck and emptying a trash container. His hand was smashed and part of a finger was amputated.

An employee was working the foot popper machine on the kill floor and amputated a fingertip on the left hand.

On or about December 30, 2015, an employee's finger was caught between the die and the die bed as the employee was rotating the die in a brake press. The right middle finger was pinched, removing the flesh part of the finger.

An employee was pulling a V-belt. The motor engaged and his right ring and pinky fingers were amputated.

An employee placed a sheet metal pipe for an HVAC duct into a roll former. His left index finger was caught between the sheet metal pipe and the die when the roller's foot pedal was actuated. The employee's left index fingertip was partially amputated.

On or about December 30, 2015, the employee was working on the plastic extruder on the production floor. The employee's right index finger was amputated after being caught in the machine while flushing the extruder. The guard was not in place at the time.

An employee was unjamming a rolling piece of equipment. When it became free it smashed his finger between the rolling equipment and another piece of equipment. The employee went to the hospital and 3/4 of the right hand ring fingertip was amputated.

Employee A was filling a cardboard baler when Employee B turned on the baler. The hydraulic drive door caught employee A's left arm and amputated it below the elbow.

An employee amputated the right index fingertip while operating a hydraulic press brake.

A mechanic was repairing a heater in a Bobcat skid steer loader. The employee was in the cab and the door closed on the employee's finger removing a piece of tissue from the left ring finger.

An employee was placing a part into a sizing press. While the employee's hand was in the point of operation, the tooling engaged and pinched the employee's right index finger between the tool and die. The employee suffered an amputation of the right index finger as a result of the incident. The light curtains installed on the press were bypassed at the time of the incident.

An employee was cutting cores with a radial chop saw and amputated the tips of the left index and middle fingers.

Maintenance employees were working on the leg quarter conveyor belt. They made and placed a steel bracket/guide above a chain like belt where it made a 20-30 degree bend around a corner to prevent the belt from popping off. While the belt was running, the injured employee saw there was a rough edge on top of the bracket/guide, so the employee reached to feel the top of the bracket/guide and his hand was pinched between the belt and the guide. His left thumb was amputated above the knuckle.

An employee was troubleshooting the box taping machine. The employee's hand was caught in the chain drive resulting in the amputation of the left thumb's distal joint. The machine was not locked out at the time.

An operator was packing out powders from the bottom spout on a ribbon blender. The gate valve on the spout became clogged. The employee reached into the spout while the blender was running resulting in the amputation of their left middle, ring and index fingers.

An employee had three fingers on the right hand crushed when the glove that the employee was wearing got caught in a gear. Part of one finger had to be surgically amputated.

On December 28, 2015 at 21:45 an employee shut down a production machine to clear a jam. The employee's left thumb was in a pinch point when a spring released pinching the employee's thumb and causing an avulsion and broken bone. The tip of the thumb was later partially amputated.

While moving a strainer onto a rack for cleaning, the rack slipped and pinched the employee's finger between the rack and the strainer weighing approximately 40 lbs. The fingertip was amputated.

An employee was changing a tire on a forklift. The tire exploded injuring the employee's right hand and resulting in the medical amputation of the right index finger.

An employee was coming out the restroom when he was struck by a moving forklift. His foot was injured during the incident which required immediate hospitalization for treatment. His left big toe was surgically amputated.

An employee's leg was amputated by a tractor-mower when it rolled over.

An employee was moving and pulling packages from the conveyor when his right hand was caught in the conveyor. His right ring fingertip was degloved.
